Application Forms

Checklist

• Have you completed all sections of the form?
• Have you made a copy of the form for yourself?
• Have you made out a cheque for GBP15.50 to JCL Consulting?
• Have you provided an email address? (This will be used to provide acknowledgement of receipt of references)
• Have you chosen up to five hospitals on your application form?  Please note, ticking 'no preference' does not allow you to apply to all of them.
